Compensation: $300 flat rate per weekend (Saturday & Sunday)
Journey of Care is seeking a compassionate, dependable Weekend Remote Care Coordinator to support Medicaid members who are at increased risk of hospitalization, institutional placement, or loss of independence.
Our members remain in their own homes while receiving ongoing remote support through a secure tablet provided by Journey of Care. Rather than replacing their healthcare providers, we work alongside them to provide consistent check-ins, care coordination, emotional support, and early intervention when problems arise.
Our team helps members navigate physical, mental, and emotional challenges while supporting medication routines, personal hygiene goals, daily wellness, and maintaining meaningful social connection through scheduled interactions.
This position is primarily an on-call weekend role with scheduled member outreach and care coordination activities. While active work currently averages 2–4 hours per weekend, applicants are expected to remain available throughout the scheduled on-call hours to respond to member needs. As Journey of Care continues to grow, both the expected workload and compensation are anticipated to increase.
Schedule
- Every Saturday and Sunday
- On-call from 8:00 AM – 10:00 PM
- Active work currently averages 2–4 hours per weekend
- $300 flat rate per weekend
- Opportunity for additional hours and increased compensation as Journey of Care grows
- Respond to member calls and urgent needs.
- Complete scheduled member wellness check-ins.
- Provide emotional support and encouragement.
- Assist members with medication reminders and wellness routines.
- Coordinate care with physicians, personal care providers, caregivers, and community resources.
- Help members navigate healthcare services and community supports.
- Document all member interactions in the electronic health record.
- Escalate clinical or safety concerns to leadership when appropriate.
Applicants must be able to:
- Maintain a dedicated, quiet, and private home workspace.
- Remain available to respond to member calls and urgent needs within approximately 10 minutes.
- Immediately transition from on-call status to active work when needed.
- Maintain reliable high-speed internet and phone access.
- Perform work without recurring interruptions from personal responsibilities or other obligations.
- Protect member privacy by maintaining a professional work environment suitable for confidential healthcare conversations.
Qualifications
- Experience in healthcare, home care, care coordination, case management, behavioral health, customer service, or a related field preferred.
- Excellent communication and relationship-building skills.
- Strong organizational and documentation abilities.
- Comfortable using technology and learning new systems.
- Ability to work independently and make sound decisions.
- Commitment to compassionate, member-centered care.
Pay: $21.00 - $35.00 per hour
